Also, delete non-essential images

Changes the behavior of the multiple_todos feature flag so that only one
todo per action type is allowed per object.

An exception is made for mentions so that users can still have multiple
mention todos since these link to different notes.

This reduces noise for the other todo actions where they link to the
same thing.

This is rendered by Nanoc and presented in an alert box in the docs
site. The second `**Note:**` can be redundant, so let's remove it.

Assures that todos targeting inaccessible epics are deleted
(either because user left the group or epic became confidential).

Also adds a background migration which checks accessibility for
all existing confidential epics.

The previous URL had two drawbacks:

1. It didn't match what the title was about (`Assignments to DevOps
   Stages and Groups`, and the anchor was `#designated-technical-writers`).
2. It didn't account for the other sections, like development and API
   guidelines.

'a to do' should always be 'a to-do item', unless
referring to the 'Add a To Do' button in the UI.
