Add Crystalball into rspec

- Configure rspec with Crystalball using custom CoverageLinesStrategy
for compatibility with SimpleCov on Ruby >= 2.5
- Running rspec with `CRYSTALBALL=true` generates crystalball data
in `crystalball/*.yml` containing files covered by each rspec example
