Distinguish Jira Connect apps by hostname · 8f332906

This change adds the GitLab hostname to the Jira Connect
app name, so we see "Create branch by GitLab (<hostname>)"
in the create branch dropdown. This was previously a problem
if there were multiple Jira Connect apps installed in the same
Jira Cloud instance.
