Form submit error in `start ultimate trial` page

The page renders a 405 error when submitting the form in start ultimate trial page in gitlab.cn, at present, we can override the haml files to temporarily hide the related elements (see the attached images) in frontend, and resume it #126 when customer.gitlab.cn is ready
